Case Study of Physiotherapy Care of a Patient after Rotator Cuff Tendon Rupture

Author: Terezie Doubková Title: Case Study of Physiotherapy Care of a Patient after Rotator Cuff Tendon Rupture Objectives: The main aim of this bachelor thesis is to theoretically elaborate the basic knowledge concerning the shoulder girdle and to develop a case study of a patient after rotator cuff tendon rupture. Another aim was to evaluate the effectiveness of my chosen physiotherapy procedures. Methods: The general part contains information on anatomy, kinesiology and problems concerning shoulder joint, obtained from the sources listed below. The developed case report of physiotherapy care of the patient includes initial and output kinesiological examination, therapy draft, therapy aims and separate therapy units, based on the knowledge acquired during my three-year bachelor studies. At the end are presented the results of the therapies and the evaluation of their effect. Results: The theoretical knowledge was written in the general part and the case report was prepared according to the chosen aims. The effect of the therapy described in the conclusion of the thesis can be evaluated as positive. Most of the measured values were improved compared to the initial examination. This bachelor thesis addresses the topic of painful shoulder syndrome. The text of the thesis is divided into two parts - theoretical and practical. The theoretical part contains the anatomy of shoulder knitting, where bones, muscles, joints, ligaments are described. It also focuses on biomechanics and kinesiology of the shoulder girdle. Furthermore, it characterizes the individual forms of painful shoulder syndrome, their diagnosis and treatment options. Last but not least, it focuses on the physiotherapy options in the painful shoulder syndrome treatment. The practical part focuses on qualitative research and its results evaluation. Data about probands was acquired via semi-structured interviews and kinesiological analysis. The research includes 3 case reports of female probands with age range between 22 - 45 years and with the diagnosis of the painful shoulder syndrome. All probands underwent the entry kinesiological analysis, 10 therapies and the consecutive exit kinesiological analysis. The research results are evaluated objectively and subjective. For two probands, there were a significant improvement and pain relief due to their regular attendance at therapies, their adherence to the home exercise and to the ergonomics of the work as well as the movement. The third proband achieved only a slight improvement and she did not pay much attention to the home exercise. The theoretical part of the thesis aimed to describe the structure of the shoulder grindle, the causes of the painful shoulder syndrome and its symptoms. The practical part aimed to structure the appropriate rehabilitation plan based on the kinesiological analysis. The bachelor thesis can serve as a professional material for physiotherapists and other medical persons, or as auxiliary study material for students of medical schools, especially with physiotherapy specialisation.
Physiotherapy for patients after stroke - problems of hemiparetic shoulder

Stroke is one of the most common causes of premature disability and is the second leading cause of death in all developed countries. It is a suddenly developing involvement of a certain district of brain tissue, which was caused by a disorder of its blood supply. One of the complications after a stroke is the painful shoulder syndrome occurring on the hemiparetic upper limb. It is pain in the shoulder area with simultaneous movement limitation. The theoretical part of the bachelor thesis deals with epidemiology, distribution, causes, diagnostics, complications and basic risk factors of stroke. Furthermore, the theoretical part presents the kinesiology of the shoulder and explains the painful shoulder syndrome at hemiparetics with subsequent treatment options. The main aim of this bachelor thesis is to map and summarize the problems of the hemiparetic arm, create a suitable rehabilitation plan and subsequently evaluate the effects of the selected physiotherapeutic treatment. This is a qualitative research, where the research group consists of 3 patients after a stroke with a hemiparetic shoulder. The case report consists of an initial entrance examination. This is followed by 8 therapies focused on the hemiparetic shoulder and finally the final exit examination. From the results is to deduce that two patients experienced some improvement after receiving physiotherapy in the shoulder area. The last patient was not able to perform an exit examination for health reasons. The work can serve as educational material for physiotherapists, professional public or students of physiotherapy who want to increase awareness of the issue.

Title: Case Study of Physiotherapeutic Treatment of a Patient with the Diagnosis of State after Fracture of Proximal Part of Humerus Objectives: To process a case study of physiotherapeutic treatment of a patient after fracture of proximal part of the humerus and following osteosynthesis with a nail. To write out complex theoretical background about stated diagnosis, about options of treatment and about following rehabilitation. Methods: The thesis is divided into two parts, general part and special part. The general part is dedicated to theoretical knowledges about stated diagnosis, which means anatomy of the shoulder joint, kinesiology of the shoulder joint, biomechanics of the shoulder joint, options in treatment of stated diagnosis and options in physiotherapeutic intervention in the state incurred after the treatment. The second, special part is dedicated to complete case study of a patient with stated diagnosis. Analysis of the shoulder girdle injury and therapy of overhead athletes in Czech republic

Jan Vocásek Thesis title: Analysis of the shoulder girdle injury and therapy of overhead athletes in Czech republic Cíl: The aim of the thesis was to create a literal research for the shoulder girdle injuries of overhead athletes, theoretically to write the anatomical and kinesiological characteristics of the shoulder girdle, to analyze the biomechanics, injury mechanisms in individual overhead sports, rehabilitation treatment and to find out how frequent the injuries of athletes in the Czech Republic follow-up treatment varies from abroad. Methods: For the purpose of this research was established a non-standardized questionnaire, based on literature studied and research questions, and then was distributed to 6 selected physicians dealing with this issue in the Czech Republic, who anonymously filled out questions regarding the shoulder joint injury based on their own clinical experiences. Results: Four of the six hypotheses were confirmed from the data obtained. The number of patients with shoulder girdle injuries in overhead sports in the Czech Republic has risen year- on-year and the most common injury is SLAP lesion. Title: The effect of shoulder girdle stabilization exercises on muscle activity during direct impact in rugby players with sub-acromial impingement syndrome Objectives: Comparison of muscle activity during direct impact while performing the rugby tackle to tackle bag and to player using amplitude analysis of electromyographic signal (EMG) before and after intervention programme for players with subacromial impingement syndrome (SIS). Application of intervention programme consisting stabilization excercises for shoulder complex and activation of deep stabilization muscles of the spine. Methods: Theoretical part contains topics about shoulder girdle, rugby and rugby injuries, mainly subacromial impingement syndrome and electromyography. Title: Case study of physiotherapy treatment of a patient after fracture of the proximal humerus treated by osteosynthesis This thesis deals with the issues of physiotherapy treatment after fractures of the proximal humerus treated by osteosynthesis. On the basis of knowledge of functional anatomy, kinesiology, traumatology, orthopaedics and by Czech and foreign literature there is an effort to clarify the time and practice schedule of post-operative rehabilitation for the above diagnosis. The work is divided into a theoretical and a practical part. The theoretical part deals with the function of the shoulder girdle, its individual structures and the influence of segmental defects of the shoulder girdle on upper limb mobility. It also deals with proximal humerus fractures and their treatment. In conclusion, it discusses the issue of post-operative rehabilitation in terms of time and practice. Title: The effect of scapular specific kinesio tape on the movement quality of the upper limb. Objectives: The aim of this study is to evaluate the effectiveness of specific kinesio tape of shoulder blade on the quality of the movement of the upper limb. Currently, kinesio taping is very popular method in the treatment of myoskeletal diseases. Kinesio taping is mostly used by athletes, however the method becomes one of the common methods in physiotherapist practice. In this diploma thesis, specific method of kinesio taping was used. The method is focused on the facilitation of the lower blade fixators and is frequently used. Positive effect of the method on the position of the shoulder joint is obvious. The movement quality of the upper limb is represented by visual motor skills coordination, which was objectified with selected testing method. The prerequisite is a close relation between the position of the shoulder girdle with the function of the hand. Centred position of the shoulder blade ensured with the fixation of the lower blade fixators allows precise movement of the entire upper limb. AABBSSTTRRAACCTT Title: Comparative analysis of selected coordinate indicators in front crawl swimming technique and crawling. Purposes: The first aim of thesis is to compare coordinate indictors of average swimming cycle and average crawling cycle. Methods: Surface electromyography of muscular activity combined with cinematography analysis used synchronized video recording. Intraindividual comparative analysis and subsequent interindividual comparison of timing muscular activation in one average swimming and crawling cycle. Results: The timing of muscular activity of m. pectoralis major and m. latissimus dorsi during swimming cycle was identical for all probands. This phenomenon was not found in crawling . Title: Function and mechanics of fascias focused to shoulder girdle Objectives: The main subject of the diploma thesis is the issue of fascial concatenation. The aim of the diploma thesis is a systematic study findings related to the selected issue and introduce concepts, that deal with the fascial concatenation. For a specific example of fascial concatenation was chosen from a practical standpoint shoulder girdle in relation to the entire upper limb. Methods: The thesis methodologically equivalent to a theoretical type of work - job search. Results: By processing the literature sources was created a comprehensive overview that relates to the topic of fascial concatenation. Attention was focused on the arrangement of fascial tissue, not only in terms of stratification, but also due to its layout. One of the chapters was focused on mechanical functions and models explaining changes in fascial tissue behavior during therapy. The thesis presented three general concepts dealing with fascial concatenation whose specific strings were introduced in the shoulder girdle in relation to the entire upper limb.
